Job Description:
The purpose of this position is to provide daily oversight and support for the Post-Acute Palliative and Homebound programs and to grow and improve the programs. This position will monitor the effectiveness of the programs as well as assist with the implementation of identified processes that improve the transition of patients across the continuum of post-acute services and the quality of care in within the post acute programs.
Accountabilities:
Directs and oversees the delivery of palliative care services within the community and the home bound program assuring quality services that meet the needs of all patients.
Assist in the development of relationships between Advocate Hospitals, PAN facilities, Primary Care Physicians and other Advocate sites of care to facilitate the transition of care for post-acute patients and to grow the palliative and homebound program.
In collaboration with the Director of Clinical services - identify and monitor data to evaluate the effectiveness of the program and the quality of patient care in the post-acute care network programs.
Directly supervises Palliative Care CoordinatorsEstimated Salary: $20 to $28 per hour based on qualifications.
