Job Description: Purpose: Assist with coordination of care for trauma patients across the continuum. Follow patient progress as an inpatient and act as a liaison for physicians and hospital staff to ensure good communication about patient plan of care. Provide continuity of care as patients transition from acute care to outpatient management by assisting with trauma clinic and outpatient management of plan of care. ________________________________________ I. ACCOUNTABILITIES AND JOB ACTIVITIES: A. Assuring/Improving Quality Care: Facilitates the patient and family's right to receive quality, cost effective care 20 %25%201. Partners with Care Management and bedside nursing staff to facilitate plan of care and keep them informed of the medical treatment plan. 6. Assist with outpatient trauma clinic in AMG Trauma office and ensure appropriate outpatient follow up for patients 7. Coordinates written communication to referring physicians and paramedic services as follow up for trauma patients. B. Communicates medical treatment plan to nursing personnel as needed 3. Communicates patient care needs from nursing to trauma surgeons as needed 4. Communicates transition plan with Care Management to help provide an effective and efficient transition to outpatient setting for patients E. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
